Claim 'missing information'

I have recently had a patient claim "unable to send due to missing information", however, we just sent one two weeks ago for the same patient. The missing information states that it is the 'relationship' within the insurance information. However....this is not the case, the insurance clearly has the relationship information in its proper place. Has anyone ever experienced this issue? We have already tried to 'drop' and reenter' all the information but it remains the same.
