2 registration forms, and med hx form

Hi someone on dental town commented on wanting registration and med hx forms, and they weren't able to share the forms there, so i attached it here
i dont remember where i originally found them..

woops i forgot that the two forms were the ones that came with open dental...one of the registration forms that are there (the one titled registration form (2) ) is the one i found somewhere, that's a little different than the open dental one...

Hi mbtoo, thanks for sharing.
I like the registration form that you posted the photo. However the 2 attachments you posted are the same with Open Dental pre-made forms.
Could you please post your personal registration form again? It looks nice!

Hi mbtoo,
I used the forms you posted. But I increased the font.. also on the birthdate.. I added (mm/dd/yyyy). Some patients were typing mm/dd/yy and open dental was giving them error message. Also under text ok?, I added (require (Please check one). And at the end, I added "Please fill out the Medical History". But I tried to upload the sheet and this board said.. the "extension is not allowed", how did you attached your file to the post?

Click on the bttom Left " Post Reply", The reply page will opem
Again , at the bottom left click on: "Upload Attachment" , a new option will open underneath it. Now you can upload your file.
Will be happy to use your form once uploaded.
