- Referral Letter forms are intended to be sent to the referring party after they have referred a patient.
Referral Slips are intended for us to use to refer our patient elsewhere.
Patient Forms require the name and date of birth of an existing patient in our database.

Web Form for Incoming Referrals
We would like to place a web form onto our website for potential referrers to enter information to refer a new patient to us. I have been unsuccessful in finding the correct type of form to do this with. From what I can find,

With that you could get creative and create a custom Patient Form that is geared towards referrals so that you at least have a way to capture their information. You could even do things like putting labels next to the FName, LName, and BirthDate fields telling the referral how to fill them out so that when you retrieve your "referral" forms they get imported into the same patient.
