I know an improvement was made in the order in which procedures list in the progress notes.
Why can't I get a extraction to show above the implant placement of the same tooth on the same day? I tried entering times (9AM for the extraction, and 10 AM for the implant), but the implant placement is still first in the notes.

Give the extraction a higher priority. You can set the priority in the tx plan module.

Yes. Give the procedure you want to go first a priority of one and the other procedure a priority of two.

Another question on progress notes:
When you right click on a note, one of the choices is Edit All. I tried choosing it, but I don't see what it does.

The 'edit all' only works on procedure codes. It is used to change the date of multiple procedures at one time (perhaps do to a charting error). Select the procedures by ctrl-clicking on each of them, right-click 'edit all', and type the new date.
