Converting New Patient Form to Sheets

To All OD Users,

As we are new with OD, we are struggling to convert our new patient registration form to Sheet/web form.. Can you advise where / how can we do that?

Jordan, is there a manual with step by step instructions to covert own form into sheets? I see lots of information scattered all over the forums in bits and pieces but not able to compile as step by step instructions to convert own forms into sheets.. This can be a real value addition to OD for users.

Please advise.

We've been putting most of our effort instead into adding more features to that part of the program to make it faster. We've shortened it from a 5 hour process to a 1 or 2 hour process. But that's for someone who does it all the time. The hardest part is converting your form into a gif or jpg. It involves having proper imaging software, and there are actually many ways to do it. It also depends on what format you're starting with. Our typical strategy is to pull it up on a huge monitor and take a screenshot. It's actually usually 2 screenshots that we then splice together. We also massage the file size until it's about 300k, but still looks crisp. It's hard to explain all that. We can do that part for you for probably about $45. Then, you could pop it into a copy of our existing registration form, and start moving things around to match. Or we can to the whole thing for you for a fee.
