Patient does not read English

I have a patient who does not read English. Can someone direct me to a place where I can import forms in Spanish into OD so I am sure he understands (in writing) the treatment he has planned? Thank you for your help. He is not a citizen, but speaks and understands English, just can't read it.

Thank you for the offer, very nice of you. I just scanned the forms into OD image but can't figure out how to post them here for you. Any idea?
