Consent Forms

I would like to be able to add other types of forms in this section.
Specifically, Warranties.
To do this, I would like to be able to to add a couple of more fields:
Recall interval and Next recall due date

How can I do this?

I guess I don't quite understand "sheets" and the difficulty of customizing them.
I'm not asking for the form to be created, I'm asking for the abreviated code so that I can customize a consent sheet.

The first practice management program I ever used, (Dentalis, DOS, used from around 1984 to '94) came with an 8.5" X 11" clear plastic grid and a list of all the codes for all the fields that the progam had (similar to the fiels in OD sheets). If one wanted to create a custom document,(recall card, delinquency letter, Insurance confirmation, patient statement, insurance claim, birthday greeting, whatever, one would just write it placing the appropriate code. If one wanted to map to a preprinted form like an ADA insurance form, one would place the clear grid over the form and enter the appropriate code in the closest grid box.

Every single "type" of sheet has a list of specific fields that can go on that sheet type. We made the decision to go with specific sheet types for now rather than some universal sheet. This allows us to do many interesting things with certain sheet types, but it also means that unusual adaptations like the one you suggest are not yet possible without us explicitly adding those fields to that sheet type. It's not a difficult request, but it is a request.

Once I saw your request, I realized that you wanted those fields within the text of a paragraph rather then at absolute positions on the page. That happens to be the next big addition to sheets, so I changed the request to reflect that. I suspect that these in-line fields will be universal across all sheets. But we'll see.
