I still see the default note getting appended after a note has been saved in a procedure.

Here is how the work flow goes in our office. Let's say I'm doing an exam, D0150. The procedure code is attached to an appointment. Our procedure codes have the framework for a SOAP note, so when I'm in the chart, I set the D0150 to complete (to generate the default SOAP note), then I hand edit it and close to save the changes. Sometimes the front desk will need to set the appointment back to treatment planned, and then to complete again, or when I have finished with the patient I go to the appointment book and set the appointment complete so that the color changes and everyone knows we're finished with that patient. Either case still generates the default SOAP note appended to my original edited SOAP note. I have found if I change line 1320 of the following code block on Procedures.cs from if(oldProc.ProcStatus!=ProcStat.C) { ProcList[i].Note+=ProcCodeNotes.GetNote(ProcList[i].ProvNum,ProcList[i].CodeNum); } if (ProcList[i].Note == String.Empty) { ProcList[i].Note+=ProcCodeNotes.GetNote(ProcList[i].ProvNum,ProcList[i].CodeNum); } The problem goes away. Similarly, changing line 3300 on FormProcEdit from textNotes.Text+=ProcCodeNotes.GetNote(provNum,ProcCur.CodeNum); to if (textNotes.Text == String.Empty){ textNotes.Text+=ProcCodeNotes.GetNote(provNum,ProcCur.CodeNum); } takes care of the same problem there.
