Access to Patient Info.gif

Is it possible to get access to the Patient Info.gif photo that is on the example registration form? I would like to use that form but change a couple of things as it would be much faster than creating my own form with all those fields.

To access it on a new form that you are creating, simply type in "Patient Info.gif" into the static image File Name box and it will show up on your new form.
It's baked into the program so if you are asking if you can export it to your desktop to manipulate it in Photoshop or something I think we'd have to email it to you. Give us a call or service@opendental.com an email and we might be able to do that for you.

Thanks for the info. I made a copy of it by duplicating the form, deleting the fields, and taking a screen capture.
