Hospital or Lab Orders

Here is my order sheet that I use for sending orders to the lab for bloodwork, orders for CT scans to radiology and pre-op orders to send to the OR or surgery center.
I have created this as a referral slip, so my process is to add the lab/radiology/OR as a "referral to" and have this as the default referral slip which can be quickly filled out and printed or email/ efaxed.
