Query for Insurance Carrier and payments by ADA code

I am looking for a query that will give the following:
Insurance Carrier....Procedure....Our Fee...Allowed fee...Ins payment
Delta..................D1110........100.00.....85.00............79.00
MetLife...............D1110........100.00.....70.00............68.00
I know it is here somewhere. I have seen it and run it in the past but can't seem to find it now that I need it.
Any help would be appreciated.
