I am trying to find the names of all patients that:
1- came to my office from jan 1 - 2011 to december 10 2011
2- HAVE DENTAL INSURANCE
3- The insurance maximum
4- The insurance amount used for that year.

the sample queries on the website are old and not functional anymore.
