Should the medical history not be dated? I would have thought this was critical for legal reasons. However no date is entered on updating a history.

For purposes of updating the Medical History, I would go to Forms and copy over the last existing MH form. By copying, that new form now has a new date. Then the patient just has to sign.
I remember another doc bringing up the fact that if you keep copying over forms like this for updating at every recall, you would get a huge list of forms. If you don't like that, you can always modify the MH form to include more lines for patients to date and sign
