medical notes on router

Where do we enter "medical notes" so that they show up on the router? I really want to be able to take the next step to paperless, but having to go through the patient's chart each time to check their medical history is tedious. I'd like to have any pertinent medical history concerns to come up on the router, which I plan on printing for each appointment even after I go "paperless." (hence the quotes around paperless since I don't ever see my office being COMPLETELY paperless)
