We've had a few requests to share a copy of the Patient Excuse Letter that is distributed with the newer trial versions of Open Dental for those users who wish to import it into their current database.
It is basic, but a good template to start with.

1. Download the file above to your local computer.
2. In Open Dental, go to Setup > Sheets
3. Click 'Import'
4. Browse to the downloaded file on your computer. Select it and click 'Open'.

Added office phone number (content says to contact office for more details but no phone number is included in form) and indented patient info with "In regards to"
