Query Help Please

I think this should be an easy query but I'm having a difficult time coming up with.
I need to find patient accounts where procedures have a secondary/other claim but no primary claim.
I've figured out how to find procedures where 2 secondary claims exists or 2 primary claims, but not where there's no primary claim.
Any help would be appreciated.
