For HIPPA reasons, the hygienists and assistants in our office like to show only the first name on the appointment. However, in Open Dental, I cannot figure out how to show the preferred name with the first name without having to show the last name.

There is not currently an option to add preferred name to the Appointment as an individual field. It is associated with PatientName, but not PatientNameF. That would be a feature request to have that option.
Even though the first name is the only text to show on the appointment is you are displaying PatientNameF, the appointment popup bubble will still display the full name of the patient, including the preferred name.

I'm not sure how that works. Do we get charged for that? Or is it just a suggestion and if you get enough people wanting that?

It's free, each office has votes that they can put towards whatever features they want. For this particular issue, you would have to create a new feature request and then once it gets approved, then you (and others) will be able to vote on it and pledge money towards it (that's the only way you'd get charged for anything).
