PSR records

where are you guys putting PSR records? I need a box with 6 sextants to punch in numbers, but i havent figured out where it is, if anywhere.

We just enter it like this 222/222

I place it in the box under the chart and in the progress note for the exam. I like to have them in the box so I can see if there's any change from one visit to the next.

gotcha, so you dont go into the perio chart at all unless its for full/sextant probing?
