FREE Google Forms - Medical History & Registration forms - Tablets, ipad - Cell

Google forms work on Tablets, Cellphones, ipad, Desktop....etc.....anything that has internet

Google forms can work on any device that has internet on it, on a cellphone, apple or android, any tablet, ipad, etc ....
No need even for a tablet, our existing patients can update their medical history, the registration form, and complete them on their cellphone

Google forms gives you the option to create and use a short URL.

They worked. Vey user friendly.
It was so simple to create these forms. There were many tutorials online.

We have been using them for 2 weeks now, and our patients like them.
We email or text the link of these forms to our new Patients, and they complete them before they arrive.

OD web forms can be used the same way as the google forms , if you increase the size of the font, less questions per page, and have multiple pages with less content in them

TUTORIALS

I am sure the forms I created can be improved. Sections could be combined together if needed. This is just my first trial.

Google forms have a customization option. We are using the basic customization now. That was enough for our office.
You can create or design in microsoft Word, save as a photo, and import in google form.
More tutorial are available on youtube. Some are advanced and some are basics tutorials. All depends on what you want to accomplish.

You can use multiple Google Form "ADDS ON" that are available online that are FREE, or you can get them at a very low cost.
With the "ADDS ON" , now you can do better designs as well as more functional forms.

With the basic google form , you can have photos, videos, etc... added to the form in just few clicks... all is explained in the basic customization tutorial on youtube. Again you can have that designed in microsoft Word , saved as a photo, and imported in google form as a question

Forms responses

1- When a form is completed online, you will receive an Email. It takes just few clicks to set that up.
* We get a pop up too on our desktop, because we use Google Voice , Hangout.

now you can see incoming mails without opening an active GMAIL window.

Save the Forms Responses in OD

We save/print the forms as pdf, and lower the size of the print out from 100% to 65% before saving the file to pdf.
The pdf file is later saved in the patient chart in the Images folder.
When the patient comes in, we print the form, ask the patient to sign it, and scan the signature page only..


SHORT URL for your forms
You can obtain the link by clicking the blue "Send" button in the top right of your form. Copy the link provided and paste it where your audience will be able to access it (Email, Edmodo, Webpage, etc.). You can also click the checkbox for a shorter URL which would allow users to type in the URL on their computer.

OPPTIONAL

2- google forms allow you to add a script to the form to take action when a form is completed, like sending email to multiple recipients when a form is completed.

Go to the Excel Format of the forms, click on tools, click on Script Editor, New Script, etc...

Google / Youtube have many tutorials on how to create a script in google forms, follow the instructions and create your own script.

The script below allows you to send yourself a text to your cellphone, and emails to multiple email addresses when the form is completed:

Tools / Script editor/ Edit/ Current Project Triggers/ + Trigger ( in the lower right) / Select event type = on form submit

it may help all users to define words to be used on these forms , so they can be synced to OD.

What's the process of syncing it into OD?

The plugin to sync the registration form with OD is not available yet - just manual entry
