Google forms work on Tablets, Cellphones, ipad, Desktop....etc.....anything that has internet

Many of us had many questions in the past about using a tablet in the reception area.
Some offices were using Surface Pro as a good tablet that allows you to use the forms that are provided by OD in the Kiosk option. ( I got one of these too) .
OD came with a better solution: using the web forms that are provided by OD, and they work nicely

After reading multiple questions and answers on this forum, going through many OD Facebook pages,
and suggested to me by Minh , a good Australian programmer for any Open Dental Project at http://www.mopensoft.com , I decided to try using Google Forms.
Google forms can work on a cellphone, apple or android, any tablet, ipad, or any device that has internet on it.
No need even for a tablet, our existing patients can update their medical history, the registration form, and complete them on their cellphone

They worked. Vey user friendly.
It was so simple to create these forms. There were many tutorials online.

We have been using them for 2 weeks now, and our patients like them.
We email or text the link of these forms to our new Patients, and they complete them before they arrive.

OD web forms can be used the same way as the google forms , if you increase the size of the font, less questions per page, and have multiple pages with less content in them

Forms responses

1- When a form is completed online, you will receive an Email.
* We get a pop up too on our desktop, because we use Google Voice , Hangout.

now you can see incoming mails without opening an active GMAIL window.

2- google forms allow you to add a script to the form to take action when a form is completed
Go to the excel Format of the forms, click on tools, click on Script Editor, etc...
Google / Youtube have many tutorials on how to create a script in google forms, follow the instructions and create your own script.
The script below allows you to send yourself an email to multiple email addresses when the form is completed:

**** Note: this script work with the fields in my form, you may have to modify it to match the fields in your form.

We save/print the forms as pdf, and lower the size of the print out from 100% to 65% before saving the file to pdf.
The pdf file is later saved in the patient chart in the Images folder.
When the patient comes in, we print the form, ask the patient to sign it, and scan the signature page only..

When you create your forms, keep in mind that OD uses special input fields.
This is a link to documents that I created, they have the input fields, Check boxes, and static fields that are used by OD...
it may help all users to define words to be used on these forms , so they can be synced to OD.

What's the process of syncing it into OD?

The plugin to sync the registration form with OD is not available yet - just manual entry
