As of October 1st, Ohio does not permit a check box for DAW designation. The check boxes are used in other states. In Ohio, the rule in OAC 4729 states that the prescriber must hand write in wet-ink “DAW” or “Dispense as Written”.
I have created a template for Rx for use in the state of Ohio. If you do not permit generic substitution, you must write "Dispense as written" or "DAW" alongside the medication you are prescribing in wet ink.

I think Medicare will use the NPI number to identify registered providers. So if you have patients that are on Medicare then you might want to add the NPI number.
