How do I import the file? I am unfamiliar with the xml format. Please explain the process.

Hello Nathan,
You can import custom sheets into Open Dental by going to Setup -> Sheets, and clicking the "import" button at the bottom of the window.

Once the traumatized tooth form has been imported, where do you go inside Open Dental to utilize it?

I believe it is an exam sheet. http://opendental.com/manual/examsheets.html
