Develping a Bridge

I am planning to write an application which enables making out going call by clicking a button. when clicked, an extension(phone) next to computer will ring and when answered it will dial the active patient. The problem is I would like to do this as external app. user can add a link to this via program links option. My issues is how to get the active patient info from my app? I would really appreciate your help.
