Potential bug in Middle Tier

Hello,

OpenDental 14.1.21.0 consistently throws an unhandled exception of type "Object reference not set to an instance of an object" when navigating to Manage>Deposits>+Add when using the Middle Tier. When clicking continue the deposits are not listed in the "Edit Deposit Slip" windows, however when connected directly to the database (Not using Middle Tier) the exception does not occur and the deposits are available for viewing. We run database maintenance to no avail. Pasted below are the details from the exception, thank you for taking a look:

I know that we have had some bugs in the past with our middle tier serialization in conjunction with language and region settings. What language/region are you using on your workstation, your server, and in the IIS settings on your server?

All the workstations, MySQL, and IIS are set to English (United States) with windows having a home location of United States.
I am checking the windows language settings from Control Panel>Clock, Language, and Region>Language
I am checking the windows region settings from Control Panel>Clock, Language, and Region>Region
I am checking the MySql settings using the query below:

Am I missing something or are these the correct places to look for the aforementioned settings?

That version of Open Dental worked fine for me around that code that you are having problems with. My guess is that there is something our database maintenance tool needs to clean up or warn you about to go manually fix. You should run DBM from your server to save time because lots of queries will be going across the wire if you run it from a workstation. If that does not fix the issue, you might have to update to a more recent version of Open Dental and run DBM from that version to see if it fixes it. If all else fails we'd need to have a copy of your database to really see what's going on under the hood.
