Customizing RX form for two different Providers

Scenario: Office has two providers with different Licensing information. Office wants to create a custom sheet so that when printing RX it prints the correct info based on selected Provider when printing the RX.

Problem: When customizing the internal RX sheet, the Output Text feilds for National Provider ID and State License Number are not an option. Besides adding them as Static Text, How can this be automated ?

Version 13.1.40 has state Rx ID and provider DEA number as output text. I haven't tried it but I believe those should work. It still doesn't have the NPI number though.
