Error on converting database from 4.4.10 to 7.0.34 (errno: 121)

Trying to upgrade from Opendental 4.4.22 to 7.0.34. I am running into an error converting the database.

I have updated the mysql database from 4.2 to 5.0 in order to make it compatible to opendental 5.0+ and also updated opendental from 4.4.10 to version 7.0.34.0. When I open opendental it asks to convert the database to the newer version (7.0.34.0) and I get the following error:

"Can't create table '.\opendental\procnote.frm' (errno: 121)
Conversion unsuccessful. Your database is now corrupted and you cannot use it. Please contact us."
