I've come across this a few times and able to replicate this easily on my Open Dental. I'm running ver 18.4.33.0.
In a patient's family module > insurance plan window, if perio maint has a frequency limit set, it will catch future procedures that exceed that limit... unless the plan is based on service year (as opposed to calendar year). Is there somewhere to submit a bug report/request?
