I haven't changed my ins claim form in a long while, but an insurance company just said they needed the 2012 form and wouldn't process my 2006 version. Where do we get the image for the claim form? It shows up blank when i go to edit the form and isn't in the "open dental\data" folder.

What version of Open Dental are you using?
The 2012 claim form is not part of the Image folder. It is distributed with the program in a .dll

Is this where you're going to get the ADA 2012 form? Because we have changed ours so the 2012 version is the default.

I compile my own...still on 15.1 right now...where would I get an image? I am not seeing it anywhere or even to buy anywhere from the ADA...

We don't distribute it anymore without an Open Dental registration key- copyright reasons.
You'll have to source your own image and add to the system. I'll dig to see if I can find a link to purchase as well.

The ADA store has two options, physical pad of 100 or PDF http://ebusiness.ada.org/productcatalog ... px?ID=1739.
If you purchase the PDF, you'll have to convert the file into an image file to use as a background.

I bought the file. Converting it and making it fit in OD is a nightmare. Do you have the exact dimensions and border sizes, etc...?
