New Patients with Insurace XYZ count

Hi All,
We are attempting to eliminate some plans at our office but want to eliminate the least popular plans first and go up from there. I'd like report that will give me a list of new patients for a given date range, the insurance plan they have (or if they are a FFS patient) and how much we been paid for that plan/patient in the same date range.
My hope is we can easily look at Q1 and see where the new patients are coming from and dump plans that don't get us too many new ones and don't pay us a ton.
Is this a custom query? I looked online and cant seem to find one that matches the criteria

I'm assuming you're in network with a number of carriers? If so, do you have separate fee schedules setup for each one? Lastly, are you really wanting a list of each patient or would a count be better?
