Txp options in OD

I have always used Auto Notes to put in a template on my periodic exam and bitewing codes that I tweak up now and then. I'm trying to make my flow more efficient as my startup gets busier. I am wondering if I could streamline some duplication of information. For example, if a person has a DO on 5 and an O on 18 I would do the following:

on D0120:
CC: None.
Medical Hx: Reviewed, no changes.
IO/EO: negative.
CE: clinical stick in O of 18.
Tx: Exam. Discussed findings w/ patient and they had no other questions.
NV: recall. CBC

on D0274:
Decay on D 5, no other problems. CBC

Then on the codes for the fillings I always write what the diagnosis is because its easier to find when I go to do the treatment or if we look back over what was previously done on a tooth rather than reading through all the various limited, new patient and recall exams. So, instead of writing the Dx in 120 and 274 codes, I am thinking of only writing it in the codes for the restoration.

Anyone see any problem with that?
