UB04 Background Image

I want to add a background image to the UB04 form. I downloaded the image from Open Dental. When I goto the Claimform setup window, I double click the UB04 form and I see that is the first item but it says that the image is missing. I placed it as indicated in the manual but still can't see it. What else do I need to do ?

You have to copy the background image to your A-Z folder. It should be .gif or .jpg. Just scan your background image in one of these formats and save it to the A-Z folder. Then you have to make a copy of the UB04 form. To do this go to Setup then Claim forms, select UB04 and click "Make a Copy". Double click the UB04 duplicate to open the editing functions. Click the ADD button and then write the name of the background image in the space on the upper left side. You have to write it with the .gif or .jpg extension. Click OK and that's it.

Yep, that will do it. I don't why the manual doesn't say like that. Thanks for the follow up.
