Progress Notes of a patient shows other patients info

Problem: Progress note in the Chart module of a patient show the progress note of other patients. See picture below:


I run an Audit trail and I also noticed that from any workstation on the network the same patient was selected: See picture below:

I also noticed that the Patient Account Comlog have a similar situation: See picture below

I run a Fix in the Database Maintenance and did notice a few errors which I have no way of knowing if this is somewhat related.

Question: What is wrong ?

It looks like all patients are in only one family. This is just a guess, but could it be that they are creating new patient records by clicking on the "ADD" icon in the Family module instead of going through the "Select Patient" button? The other thing that comes to mind is if this could be the result of a faulty data conversion.

The latter sounds more like it. I checked to see if there were any additional family member but he is all by himself.

Looks from the date formats like you are from another country where HIPAA isn't an issue. Otherwise, I was going to insist that you remove those screenshots.

Anyway, what version are you using? Did we recently do a conversion from another software? Are you using any third party software that might be involved with appointments?

Jordan, good catch. Now that you have a good mental picture . The answer to your question is yes. This was a data migration. The version is 12.1.14 . No this is USA. No there is no 3rd party. Lastly, is there something that can be done to verify that this doesn't happen again ? Thanks for your observation.

So could it have been clutter from the original conversion? In that case, it wouldn't happen again anyway.
