Production report

We are using version 5.1 and I just noticed when we print our a production report that some of the patients names are missing. It shows the ada code, tooth number, descp, doc, and fee but no patients name. This only occurs on one or two patients a day, all the rest of the patient's names show correctly. Is there a known problem with this report?
