ICD-9 Diagnosis does not show up on health insurance claim

I am trying to enter in the ICD-9 code into the Health Insurance Claim Form. I have followed the directions from the help file but it still does not show up on the health insurance form, the only thing that shows up is the diagnosis code which is 1,2,3 (according to how you ordered the ICD-9 code in each procedure). I tried putting the ICD-9 code in each procedure in the claim, but i only get the diagnostic code. If someone could help that would be wonderful. Thanks in advance.

Additional info: This is for the diag codes on field 21 of the HCFA-1500 form.

Try our "1500" claimform instead of our "HCFA-1500" claimform. It's downloadable from our medical insurance page.
