Scan my form into sheets

Is it possible to scan my own medical history form into sheets instead of making a new one?
Is it possible to get my own medical history form onto my website the same way?

Ok, then how can I do it and what file does it have to be jpg, pdf??

jpg or gif both work. You'll get the cleanest image if you use a screenshot from a print preview of your original Word document. An alternate way to clean up the image is to scan it in high resolution, then resample it in Photoshop or Corel Photopaint. You can also use that software to clean up any specks and smudges. Make sure the file size isnt' too big. 300 kB is a nice size. If you have never worked with images, then it might take you most of a Saturday to come up with a decent image. You could also just let someone else do it. We can do it for $45/hr, and it would probably takes us less than an hour.

I am tryin to set up a custom sheet. I am developing the chart in Illustrator because it is easier and then adding misc check boxes in open dental. I am saving it as the highest quality .gif, .jpg, and .png. Why does this ipmorted images always have that grainy appearance? Am I missing something here? I did see where importing a .png left the image at its normal size. .jpgs seem to be automatically smaller and when you enlarge them in open dental they get blurry. Any suggestions

You're just talking about the sheet, right? You haven't even started trying to upload it as a webform, right? When you "import" an image, all it's doing is making a copy of the image file in the SheetImages folder in your AtoZ folder. So Open Dental definitely doesn't make it blurry as part of the import process. An image normally has "scaling" properties, also known as dpi. We ignore those totally, only paying attention to how many pixels it has. So a 5"x7" image that is at 200 dpi is acutally a "bigger" image in terms of pixels than a 5"x7" image that is 100 dpi. So your jpg shows up as smaller because it had less pixels to start with. I don't know where it came from, but if it started out with a large dimension, then it must have been at low dpi. Scan at a higher dpi.

Where could one find the internal image files that you all have used in the sheets. I need the patient information gif so that I can crop it and just as another section to it. Any info would be great.

look in your shared folder, what we call the A-z folder, there is a sub folder called SheetImages. It is in there.
