How do you change the total daily production shown on appointment tool bar to include Hygiene and Dr. Production?

This is controlled by the "Appointment View" feature. You select the appointment view you want a couple of lines up from where the production shows in the appointment module. The answer to your question is likely found by going to Setup| Appointment Views. It is here where you can create different appointment views. For example, we have a view just showing doctors, another view showing only hygienists, the default view showing all providers , etc. In the Appointment View setup, you can decide which providers to include, which operatories and what information shows on the schedule in each view. The providers that are picked for each view will determine what production is shown in the box you referred to in your question.
There are many uses for Appointment Views. I'll give you a couple of examples. As noted above, you can have a view just showing hygienists which is very helpful if you are searching the schedule for a hygiene opening. You can have a view that shows all the patient's contact phone numbers. This is helpful if you are making appointment confirmation calls.
