Registration Form Sheet

There should be an available field for the Subscriber's birthday, for both primary and secondary insurance:
ins1SubscriberBirthday
ins2SubscriberBirthday
Need it for those cases where the subscriber is not a patient and their information is not in OD already. In order to file insurance the subscriber has to be added to OD as either a patient or non-patient, and the birthday is needed.

It should be a misc field. You can still have any field at all on the form, even if we don't explicitly support it.

I guess I could set it up as a misc field. Is it a problem to add another birthday field? You already have ten (times 2) other insurance related fields, what's two more?

Because those are import fields. If there's nothing to import, there's no point in having a named field.
