I have been entering lab cases through the chart view. But this does not coordinate with the lab case (and upcoming appointment) that is set up in the appointment view. Almost like they are two separate records. Am I doing anything wrong here?

I was doing the same thing. You need to attach the lab case to the appointment. Click on the appointment you want it to be attached to, click on the lab button (middle left) and either create a new lab case or choose one already there.

I train our users to always create the lab case from the planned appointment rather than from the toolbar. That way it is always attached.
