Procedure count for past year ?

How can I determine a count of each procedure that I have done in the past year ?

For example, how can I determine how many one surface posterior composites I did, how many anterior endos, how many routine extractions, etc ?

I want to get a list of my most commonly performed procedures so that I can make a spreadsheet listing my fee and the UCR's of each insurance company that I deal with.
