Sheet Request - Letter to Insurance Company

Request for additional fields on the Letter Sheet

A letter type sheet with insurance company information would be very helpful when writing concerning procedures, denials, etc.

So the sheet would need - Insurance company info, Pt info including subscriber # - (if SS# is there a way to block out all bu the last 4?) and of course practice info at the top.

It would be pretty difficult to select a procedure, so most of the text would be free-form.

Carrier name and address and subscriber name and ID# are already available in the list of Static Text fields.
