Jordan...

I am getting ready to create my registration forms and medical history forms but wanted to ask a question before having to go back and remake them later.

If I put a signature box on my medical history form and registration form how will the signature box be treated online?

(1) Will it just be deleted from the online webform that the patient sees so the patient is not confused about what should be entered in that spot?

(2) Can I just open the completed webform when the patient next shows to the office and have them sign it then?

(3) How is the date on the medical history handled? For example the patient fills out the webform 2 weeks before their appointment then comes in two weeks later and signs the form with the old date. Can the date be automatically or manually updated when they sign it later?

Just shows as a rectangle that says something like, "signature will be collected later". You could remove it from your sheet before uploading it, or you could have the patient sign it at the office. So:
1. No.
2. Yes.
3. I think you can manually update it.

There is no online mechanism for that yet. The web forms are for new patients at this point. One thing that we have avoided for now by doing it this way is that we never risk putting private patient info out on the web. The kiosk feature works well for updating in the office. Use it with the copy feature.

Is this on version 7.3?
Because after "setup" there is no "add" button.

Jordan, I don't think we need the ability to update online, if I'm following you. We've tried the kiosk feature for med histories, but the background images don't copy over and we wind up with a new med history with every med history update. Thus, for same patients, I have 6+ med histories showing up.

I know I keep beating the proverbial horse, but I think ultimately we need a med history form that is linked to the database. Furthermore, with each update, OD would automatically archive the most recent copy. We don't need multiple old med histories showing up, just the most recent. but certainly need the archived copies for legal purposes.

Heck, if Lexicomp could be integrated with the medical history that would be wonderful. Start typing the first few letters of a drug and choices pop up. I think that would help patients, staff, and myself. Much like texting on a cell phone. For instance, I type "amo" and "amoxicillin" automatically pops up.

What version are you using? The background images don't copy over? What do you mean?

The copy button that I was talking about was added in 7.4. Are you familiar with how it works?

Yes, integration with Lexicomp is coming as soon as we can possibly get to it.

We're on version 7.5. I just tried it and the background images do copy over. We quit using the feature previously because background images weren't copying over. Regardless, we still wind up with multiple health histories and it becomes a lot to sort through.

I think we're gonna love webforms.

But those health histories are all ordered by date in the patient forms window, right? So can't you just look for the last one in the list?

Yes, we can. However we update @ every visit. For some patients we are literally getting 10+ health histories showing. Give it another 2-3 years and we'll have 20-30 health histories in the list. Rather than do that, we decided to stay as is and wait for a health history that is linked to the database. It really didn't bother me, but it bothered the staff, so we reverted back to the way we were.

I still don't understand. If you update it at every visit, then you are going to have 10+ health histories showing no matter how we formalize it in the future. The whole point of doing an update is that you document it, and that means another row. So maybe all but the current would be hidden in the patient forms window by default. But I think that every single one of them should show in the Progress Notes because they are a legal part of the clinical record. All but the most recent might say "archived" or something.

For most patients, their medical history doesn't change at every recall interval. I think asanders was just stating that it doesn't make sense to have 10+ forms of the same thing. It would be better to have 1 form, but with say...5 lines for signatures/dates denoting that nothing has changed. I personally don't see a problem with clicking copy and having the patient sign/date it, but some docs would rather have 5 signatures/dates on the same form. (5 is meant as an arbitrary number).

I'm trying hard to think of ways to formalize this. In the meantime, if you are doing that many updates, why not add 5 signature boxes to your form?

Jordan, I've been a very happy user of MedicTalk Dentforms for the last couple of years; if I am understanding this correctly, however, it seems the new OD Webforms would remove the need for MedicTalk?
