How do we use OD to print school excuse slips with all patient/appointment info.?

That needs to be changed on the ortho page. That's already possible. Use a patient letter. It's a kind of sheet. The letter will mostly be one big static text field. Within that box, you can have in-line fields. Like this:

To whom it may concern:
Please excuse [nameFL] from school on [dateToday]. [nameF] had a dental appointment with Dr. Smith.

I'm sure you could do much better with the wording.

I highly recommend using the sheets framework. I think our new trial version has a built-in "patient excuse form" so you might want to check that out and consider using it as a starting point for your school excuse form. The sheets framework is getting more and more robust (in the middle of adding a 'mobile' version of sheets specifically tailored to phones and tablets) and the letter merge tool isn't being actively worked on. My personal suggestion would be to use a "PatientForm" sheet type as your core starting point, it tends to have the most available input / output text fields in regards to patient information.
