Hi
My OD 5.0.21 doesn't print the phrase "Signature On File" in box 53 of ADA2006 Claim form even though I already checked the "Signature on File" option in Provider setup.
I know I can Photoshop directly into the ADA2006.gif but this is not a good idea. The other solution is the change the value of the field "Treating Dental Signature".
How do I do this?

Do you have the Assignment of Benefits box checked in the InsPlan?

Some things have changed with that box in recent versions. You now have a choice of showing "signature on file" or of showing the provider's name. That's in the module setup. So I'm going to suggest that the easiest thing to start off with would be to upgrade. I can't guarantee for sure that it will solve your problem, but we try not to troubleshoot older versions since it's usually not a good use of our time.

a search on my C drive could not find ADA2006.gif file.... do you know where is the ADA2006.gif located? Thank you.

i think it is in the data directory (your A-Z folder)

It's compiled into the exe for security reasons. If you have the version that we release, you can just refer to that file and it knows to look inside the exe instead of in the AtoZ folder.
