No background image for Denti-Cal claim

We have downloaded the gif and xml files to the a-z folder (forms) and imported the xml to the claims setup. When we view a claim, there is no background image. We have done everything in claims setup recommended in the manual and on the forum - even made a copy and tried adding the gif, but the message still reads "no image found". We have tried renaming the gif and moving it to different folders, and still nothing. What is the problem? Thank you-

Are you storing images in the database or using the AtoZ folders? To verify, go to Setup | Data Paths | A to Z Images Folder for storing images and documents.

The file name of the image must exactly match the item name in your claim form. The image must reside in the root of the AtoZ Images folder, not within a sub-directory of the share. To know your root AtoZ folder path, simply go to Setup | Data Paths and it will be the path that displays in the top most text box.

Also, you could double check to make sure your image on the claim form has a defined Width and Height (gif images will not draw without them. I believe jpgs will always draw regardless).
