Hygiene commission

Post by Hersheydmd » Sat Sep 03, 2011 6:59 am

I just started paying one of my hygienists on commission instead of hourly.
Normally when I do an exam (D0120 periodic exam or D0150 comprehensive exam) the procedure is assigned to me and she isn't paid on it.
The problem is with a Comprehensive Perio Exam (D0180), where she spends time doing the probing and I examine and diagnose the patient. I would like to give her credit for 60% of the fee, without making things too complicated. If I split the fee between two different procedures the insurance companies won't know how to handle it. I can't use adjustments because they won't show up when I generate a report of her production for the pay period.
Anyone have any ideas on how to handle this?

Use a couple of custom reports. One report will show everything except those two codes. The other report will only show those two codes.

I just discovered that you can assign a procedure a negative fee which led me to a cool solution.

CompPerioExam = D0180 = $140 - IsHygiene box not checked (so the procedure and fee are assigned to me)
Probing = N0180 = $ 84 - IsHygiene box is checked (so it is assgined to the hygienist); DoNotBillToInsurance is checked so it doesn't get added to the insurance claim.
Probing credit = N0181 = $-84 - IsHygiene box is not checked (so the $60 is deducted from my production); DoNotBillToInsurance is checked so it doesn't get added to the insurance claim.

I grouped the three codes together in the Appointment Procs Quick Add window and in the Single Click Procedure Buttons (in the Chart window).
Using the Quick Add feature, adding "CompPerioExam" to a hygiene appt adds all three "procedures" and automatically assigns the Probing to the hygienist ($84) and the Exam to me ($140-84 = 56). It shows up in the ledger as:
Comprehensive Perio Exam $140 (gets billed to insurance)
Probing $ 60 (NoBillins)
Probing credit $-60 (NoBillins)

Now when I generate a simple production report for just the hygienist her production total is correct without doing any additional calculations.

SWEET!
