You can show these in the chart by playing with the "movements" tab. It will end up showing the teeth overlapped a little in the chart, but it's better than nothing. As far as I know, you would only want to use an allowed fee schedule of you are out of network but somehow still have access to the allowed fee schedule for that insurance company. For a PPO, you would enter the contracted fee schedule as a normal fee schedule, then under the insurance plan edit win...

Does this need to be put in as a feature request? I can't see any reason why you would only want the first name of the subscriber. We have many families where the subscriber has a different last name than their children on their plan.
