Walkout statement

We just start using OpenDental and I need to change the way "Walkout Statement" looks like.

I'm trying to eliminate all the treatment details (code, tooth, description) it is not needed on the walkout statement.
Does anybody know which file is it? Is it possible to modify it?

OD is very easy on hardware so don't get too carried away. A server is best but is you have fewer than 10 computers XP will function as your server. A lot of people use a Linux server.
